The objective of this study was to develop and validate a noncontact monitoring system for respiratory rate variability in rats under anesthesia using a 24GHz microwave radar sensor. This study aimed to address the need for stress-free monitoring techniques that comply with the 3Rs principle (Reduction, Replacement, and Refinement) in laboratory animal settings.
Methods
Utilizing a 24GHz microwave radar sensor, this system detects subtle body surface displacements induced by respiratory movements in anesthetized rats. The setup includes a 24.05 to 24.25 GHz radar module coupled with a single-board computer, specifically Raspberry Pi, for signal acquisition and processing. The experiment involved four male Wistar rats tracking the variability in their respiratory rates at various isoflurane anesthesia depths to compare the radar system’s performance with reference measurements.
Results
The radar system demonstrated high accuracy in respiratory rate monitoring, with a mean difference of 0.32 breaths per minute compared to laser references. The Pearson’s correlation coefficient was high (0.89, p < 0.05), indicating a strong linear relationship between the radar and reference measurements. The system also accurately reflected changes in respiratory rates corresponding to different isoflurane anesthesia levels. Variations in respiratory rates were effectively mapped across different anesthesia levels, confirming the reliability and precision of the system for real-time monitoring.
Conclusion
The microwave radar-based monitoring system significantly enhanced the animal welfare and research methodology. This system minimizes animal stress and improves the integrity of physiological data in research settings by providing a non-invasive, accurate, and reliable means of monitoring respiratory rates.

Laboratory animals, 236772241265541-236772241265541, 11 Sep. 2024, Peer-reviwed, True, The aim of this study was to verify the possibility of obtaining vital sign information using a laser and radar sensor in a manner that is non-invasive and painless for test animals. A dataset was obtained from respiratory movement of anaesthetized male F344 rats, signals of laser and radar sensors were recorded simultaneously with vital data acquired with an integrated multiple-channel intraoperative monitor. In addition, respiratory movements were also video recorded, and used as reference data of respiration rate (RR; ref-RR). Reference data for heart rate (HR; ref-HR) were obtained from the R wave of electrocardiogram data for each epoch. Signals recorded from the radar sensor (I- and Q-signals) were input to a computer, and HR (radar-HR) and RR (radar-RR) were estimated using the frequency analysis method. Among the six positions where respiratory movements were measured by the laser sensor, the number of peak counts matched the visual counts of respiratory movements in the video records. The respiratory movements were significantly the greatest over the most caudal rib in the dorsal (p < 0.001). The average radar-RR and ref-RR values showed correspondence (ref-RR, 69 ± 6.2 breaths/min; radar-RR, 68 ± 5.7 breaths/min (p = 0.04-1.00); equivalence ratio, 86%). The radar-HR data showed slight variability; however, there was 80% homology compared with the ref-HR values (ref-HR, 336 ± 19.6 beats/min; radar-HR, 348 ± 34.1 (p = 0.10-0.95)). Although comparison of the data under noradrenaline administration failed to track drug-induced changes in some cases, the HR and RR data of anesthetized rats measured from the radar sensor system showed comparable accuracy to other conventional methods.

The focus on deriving cardiac interbeat interval and heart rate variability has gained significance due to its complexity and relevance in healthcare. Our investigation involved a detailed analysis of continuous wave radar signals to enhance the extraction of chest wall movement data. Using a convolution algorithm, we eliminated the respiratory component from the signal, while a locally projective noise reduction algorithm helped isolate the heartbeat component. Subsequently, a derivation filter was applied to pinpoint the R peak of the heartbeat, facilitating the collection of IBI and HRV metrics. This methodology proved effective for individuals in a relaxed, motionless state. However, its efficacy in cases of elevated heart rates caused by factors such as exercise or caffeine consumption remained uncertain. For subjects with large changes in heart rate followed by large changes in cardiac IBI, we made a small improvement in the algorithm. By performing a window shift of 10 seconds with an overlap of 1 second. Each 10-second data segment is fed into the algorithm. At each data segment, we perform multiple iterations with decreasing number of neighbors until no further change is made. With this adjustment, the results achieved in the group of subjects using input stimulation to increase heart rate such as exercise, or drinking coffee were indicated a strong correlation of 96.38% between radar-based measurements and reference measurements for this group of subjects, affirming the effectiveness of the proposed method in such scenarios.

Respiratory rate (RR) is known to be a more accurate predictor of clinical deterioration than other vital signs. However, there were few respiration measurement devices certified as medical devices that could be used in daily clinical settings. Therefore, using a bult‐in microcontroller (72×57×12 mm) instead of a personal computer, we developed a portable stand‐alone respiration measurement device with minimum workload in computing that can be used for non‐contact measurement in 30 s using a Doppler radar. In this study, the problems of respiration measurement using a Doppler radar, such as miscounting of respiratory peaks were clarified, and proposed device with respiratory peaks miscount prevention algorithm achieved high accuracy RR measurement. Clinical testing was conducted on pediatric outpatients of Children's Medical Center, Showa University Koto Toyosu Hospital. The measurement accuracy of the system was confirmed to be comparable to the respiration measurement accuracy of stationary certified medical devices used in hospital, such as capnometers and the chest wall impedance method used in bedside monitors.

The present study aimed to assess the usefulness of heart rate variability (HRV) indices. Frequency-domain HRV indices, including high-frequency (HF) and low-frequency (LF) components, their sum (LF+HF), and their ratio (LF/HF), were measured in a three-behavioral-state paradigm composed of initial rest (Rest), task load (Task), and post-task rest (After) periods to examine autonomic regulation. It was found that HF was low at Rest in both disorders, but was lower in MDD than in CFS. LF and LF+HF at Rest were low only in MDD. Attenuated responses of LF, HF, LF+HF, and LF/HF to task load and an excessive increase in HF at After were found in both disorders. The results indicate that an overall HRV reduction at Rest may support a diagnosis of MDD. HF reduction was found in CFS, but with a lesser severity. Response disturbances of HRV to Task were observed in both disorders, and would suggest the presence of CFS when the baseline HRV has not been reduced. Linear discriminant analysis using HRV indices was able to differentiate MDD from CFS, with a sensitivity and specificity of 91.8% and 100%, respectively. HRV indices in MDD and CFS show both common and different profiles, and can be useful for the differential diagnosis.

Computer methods and programs in biomedicine, 226, 107163-107163, 27 Sep. 2022, True, BACKGROUND AND OBJECTIVE: Continuous monitoring of vital signs plays a pivotal role in neonatal intensive care units (NICUs). In this paper, we present a system for monitoring fully non-contact medical radar-based vital signs to measure the respiratory rate (RR), heart rate (HR), I:E ratio, and heart rate variability (HRV). In addition, we evaluated its performance in a physiological laboratory and examined its adaptability in an NICU. METHODS: A non-contact medical radar-based vital sign monitoring system that includes 24 GHz radar installed in an incubator was developed. To enable reliable monitoring, an advanced signal processing algorithm (i.e., a nonlinear filter to separate respiration and heartbeat signals from the output of radar), template matching to extract cardiac peaks, and an adaptive peak detection algorithm to estimate cardiac peaks in time-series were proposed and implemented in the system. Nine healthy subjects comprising five males and four females (24 ± 5 years) participated in the laboratory test. To evaluate the adaptability of the system in an NICU setting, we tested it with three hospitalized infants, including two neonates. RESULTS: The results indicate strong agreement in healthy subjects between the non-contact system and reference contact devices for RR, HR, and inter-beat interval (IBI) measurement, with correlation coefficients of 0.83, 0.96, and 0.94, respectively. As anticipated, the template matching and adaptive peak detection algorithms outperformed the conventional approach. These showed a more accurate IBI close to the reference Bland-Altman analysis (proposed: bias of -3 ms, and 95% limits of agreement ranging from -73 to 67 ms; conventional: bias of -11 ms, and 95% limits of agreement ranging from -229 to 207 ms). Moreover, in the NICU clinical setting, the IBI correlation coefficient and 95% limit of agreement in the conventional method are 0.31 and 91 ms. The corresponding values obtained using the proposed method are 0.93 and 21 ms. CONCLUSION: The proposed system introduces a novel approach for NICU monitoring using a non-contact medical radar sensor. The signal processing method combining cardiac peak extraction algorithm with the adaptive peak detection algorithm shows high adaptability in detecting IBI the time series in various application settings.

Frontiers in Physiology, section Computational Physiology and Medicine, -, 905931--, Jun. 2022, Peer-reviwed, True, Background: To conduct a rapid preliminary COVID-19 screening prior to polymerase chain reaction (PCR) test under clinical settings, including patient's body moving conditions in a non-contact manner, we developed a mobile and vital-signs-based infection screening composite-type camera (VISC-Camera) with truncus motion removal algorithm (TMRA) to screen for possibly infected patients. Methods: The VISC-Camera incorporates a stereo depth camera for respiratory rate (RR) determination, a red-green-blue (RGB) camera for heart rate (HR) estimation, and a thermal camera for body temperature (BT) measurement. In addition to the body motion removal algorithm based on the region of interest (ROI) tracking for RR, HR, and BT determination, we adopted TMRA for RR estimation. TMRA is a reduction algorithm of RR count error induced by truncus non-respiratory front-back motion measured using depth-camera-determined neck movement. The VISC-Camera is designed for mobile use and is compact (22 cm × 14 cm × 4 cm), light (800 g), and can be used in continuous operation for over 100 patients with a single battery charge. The VISC-Camera discriminates infected patients from healthy people using a logistic regression algorithm using RR, HR, and BT as explanatory variables. Results are available within 10 s, including imaging and processing time. Clinical testing was conducted on 154 PCR positive COVID-19 inpatients (aged 18-81 years; M/F = 87/67) within the initial 48 h of hospitalization at the First Central Hospital of Mongolia and 147 healthy volunteers (aged 18-85 years, M/F = 70/77). All patients were on treatment with antivirals and had body temperatures <37.5°C. RR measured by visual counting, pulsimeter-determined HR, and BT determined by thermometer were used for references. Result: 10-fold cross-validation revealed 91% sensitivity and 90% specificity with an area under receiver operating characteristic curve of 0.97. The VISC-Camera-determined HR, RR, and BT correlated significantly with those measured using references (RR: r = 0.93, p < 0.001; HR: r = 0.97, p < 0.001; BT: r = 0.72, p < 0.001). Conclusion: Under clinical settings with body motion, the VISC-Camera with TMRA appears promising for the preliminary screening of potential COVID-19 infection for afebrile patients with the possibility of misdiagnosis as asymptomatic.

Sensors, -, 15, 2021, Peer-reviwed, True, Using a linear discriminant analysis of heart rate variability (HRV) indices, the present study sought to verify the usefulness of autonomic measurement in major depressive disorder (MDD) patients by assessing the feasibility of their return to work after sick leave. When reinstatement was scheduled, patients' HRV was measured using a wearable electrocardiogram device. The outcome of the reinstatement was evaluated at one month after returning to work. HRV indices including high- and low-frequency components were calculated in three conditions within a session: initial rest, mental task, and rest after task. A linear discriminant function was made using the HRV indices of 30 MDD patients from our previous study to effectively discriminate the successful reinstatement from the unsuccessful reinstatement; this was then tested on 52 patients who participated in the present study. The discriminant function showed that the sensitivity and specificity in discriminating successful from unsuccessful returns were 95.8% and 35.7%, respectively. Sensitivity is high, indicating that normal HRV is required for a successful return, and that the discriminant analysis of HRV indices is useful for return-to-work screening in MDD patients. On the other hand, specificity is low, suggesting that other factors may also affect the outcome of reinstatement.

We performed regression analysis considering relationship between time of one heartbeat cycle and systolic blood pressure. The time of one heartbeat cycle was acquired using PPG and Doppler radar. Since the obtained regression equation included individual differences, we obtained a regression equation that does not include individual differences by using the time of one heartbeat cycle and the systolic blood pressure at rest as individual parameters. Comparing the blood pressure estimated using this regression equation with the reference blood pressure, we obtained the correlation coefficient of 0.9, which was sufficiently high. The potential of non-contact blood pressure measurement using Doppler radar is also shown.

Using transient responses induced by multiple mental tasks that random number generation (RNG) task and verbal fluency (VF) task, the system discriminates MDD patients from normal subjects using heart rate variability (HRV) indices and cerebral blood flow determined by near-infrared spectroscopy (NIRS). The screening was conducted on twelve MDD patients (32±15 years) and 30 control subjects (37±17 years). The mental work load procedures are composed of "Pre-task rest", "Undergoing task", and "After-work rest" sections. When we adopted only RNG task, the system indicated 83.3% sensitivity and 93.8% specificity and with only VF task, the sensitivity and specificity were 75% and 90%, respectively. However using multiple tasks, we were able to obtain 100% sensitivity and 100% specificity with our proposed system.

Transactions of Japanese Society for Medical and Biological Engineering, Japanese Society for Medical and Biological Engineering, 55, 3, 162-162, 2017,After an outbreak of severe acute respiratory syndrome (SARS) in 2003, infrared thermography (IRT) systems were used as the border control strategies for fever screening of passengers at most major international airports. In this study, we proposed a visible/thermal image processing approach using only a CMOS camera-equipped IRT, thereby remote sensing of multiple vital signs for the rapid and accurate screening. Due to infection screening system expensive and required large-scale system structure, the system still has not reached yet the stage of widespread use. In this regard, we focused on minimum the hardware requirements to achieve a system that more suitable for real-world settings. The system displays the results via the logistic regression discriminant function derived from the heart and respiration rate, and body temperature within 10 seconds. The system achieved a sensitivity and specificity of 87.5% and 91.7%, which is notably higher compared to conventional fever based screening.

For a mobile robot in the real world, there are several types of uncertainties for its perceptions, such as the ambiguity between a target object and the surrounding objects and occlusions by furniture. The problem could be more serious for a home behavior-monitoring system, which aims to accurately recognize the activity of a target person, in spite of these uncertainties. It detects irregularities and categorizes situations requiring further explorations, which strategically maximize the information needed for activity recognition while minimizing the costs. Two schemes of active sensing, based on two irregularity detections, namely, heuristic-based and template-matching-based irregularity detections, were implemented and examined for body contour-based activity recognition. Their time cost and accuracy in activity recognition were evaluated through experiments in both a controlled scenario and a home living scenario. Experiment results showed that the categorized further explorations guided the robot system to sense the target person actively. As a result, with the proposed approach, the robot system has achieved higher accuracy of activity recognition.

To classify people into severe-influenza patients, mildly-influenza patients and healthy people at places of mass gathering, we developed an influenza screening system. The system conducts screening within 10 s from vital-signs, i.e., respiration rate, heart rate, and facial temperature. A neural network based discriminant function was implemented into the system to predict the infected individuals. We conducted influenza screening for 35 seasonal influenza patients at the Japan Self-defense Central Hospital. To assess the clustering performance of this system, SpO2 was measured as a reference. The system classified 34/35 influenza patients to be mildly or severe influenza. The 10/17 severe-influenza group indicated SpO2 less than 96%, while, only 2/17 mildly-influenza group showed SpO2 less than 96%. This result indicates that the system has the potential to serve as a helpful tool for rapid screening of influenza in clinical settings at places of mass gathering.

Radar technology, particularly Frequency-Modulated Continuous Wave (FMCW) radar, has garnered attention in the field of smart home monitoring due to its high sensitivity, long-range surveillance capabilities, and privacy-preserving characteristics. This study proposed a human motion state recognition system to determine motion state based on the spectrums form radar signals in home environment. A target’s velocity is assessed via micro-Doppler signature, compare against a velocity threshold, while its height signature discerns between high and low postures against a height threshold. The target’s horizontal position is determined via the Range-Angle map. The proposed threshold algorithm defines the motion state on velocity, height postures and indoor position. Subsequently, the performance of the system was evaluated by conducting experiments on three subjects. 在宅看取り支援の一環として,終末期の徴候で,呼吸振幅が周期的に増減するチェーンストークス呼吸(Cheyne-Stokes respiration:CSR)に着目し,療養型病院においてCSRの自動検知に成功した(Y.Otake. et. al., Eur Heart J Case Rep.2021).2台のドップラーレーダー(24GHz,10mW)をベッドのマットレス下に設置して呼吸波形を求め,呼吸波形の包絡線の振幅のパワースペクトル密度(PSD)の最大値が0.0083~0.033Hz(日本救急医学会)の範囲に存在した場合CSRと判定したが,体動による呼吸振幅の見かけ上の変動に起因する誤検知が多く,特異度が低い値を示した.本研究では以下2つのアルゴリズムを提案し,CSR検知精度向上を目指した.① PSDの高周波ピーク(0.033Hz~)を用いた体動検出アルゴリズム. ②CSRに付随する無呼吸(睡眠障害国際分類ver3)検出アルゴリズム.①と②のアルゴリズムを搭載した新システムを用い,2021年4月までに元気会横浜病院に認知症,脳出血後遺症等で入院した22名の患者の連続モニターを行った.22名中3名(女性3名,77±10歳)で110回のCSRイベントが発生し,感度92%(92%),特異度99%(79%)(①②無し),正確度96%でCSRを自動検知できた.在宅での運用を予定している., 公益社団法人 日本生体医工学会, 2022, 生体医工学, Annual60, Abstract, 136_2-136_2, Japanese, 1347-443X, 1881-4379, 202202270046608619 - スマートフォン内蔵の深度センサを用いた非接触呼吸機能計測アプリ

肺機能計測において,個人レベルでの即時・常時計測を可能にするためには,場所と時間を問わず簡単に利用できるシステムが必要であると同時に,臨床診断にも十分に利用できる必要がある.この要求を担うプラットフォームとしてモバイルヘルスが注目されている.呼吸器疾患では特定の領域に発症するものがあり,左右・上下の呼吸運動に非同期性が現れる.よって左肺と右肺の呼吸信号は呼吸器疾患の兆候・進行・改善を診断する上で重要なインジケータとなる.本研究では肺機能計測システムをスマートフォンとその内蔵深度センサを用いたアプリとして実装する中で,生理学に基づく胸部ROI分割手法を2つ提案し,信頼性の高い左右の呼吸信号推定を行う.本分割方法はセンサで計測された2次元深度マップにおいて,吸気による胸腹部前方の変位量に各領域で差が表れることを仮定する.実装したシステムの評価結果において,ROI分割は被験者3名全てで成功し,ROIを分割できた場合の各領域の変位量には分割できなかった場合と比較して有意な差(p<0.05)が確認された.同時計測した左右胸郭の生体インピーダンスに対して,分割ROIから計測した左右胸部の推定呼吸波形それぞれが高い相関を持つことが確認された(r=0.94).本システムによって,スマートフォンのみを用いた左右呼吸信号の即時・常時計測が可能になる., 公益社団法人 日本生体医工学会, 2022, 生体医工学, Annual60, Abstract, 136_1-136_1, Japanese, 1347-443X, 1881-4379, 202202219108059286 - RGBカメラを用いた非接触酸素飽和度計測の基礎的検討

動脈血酸素飽和度(SpO2)は,身体の異常を検知する重要な指標である.SpO2計測には,生体を透過した光を利用した接触式のパルスオキシメータが実用化されている.接触式の問題点として,長時間計測による患者への不快感を起こす可能性がある.本研究では,非接触式RGBカメラからの容積脈波(imaging-Photoplethysmography,iPPG) 信号を抽出し,非接触でSpO2計測の可能性を検討する.抽出されたRGB3チャンネル信号の異なる光の波長における酸化ヘモグロビンと還元ヘモグロビンの吸光スペクトルの違いを利用して,SpO2を推定する.容積脈波信号は,動脈血の脈動成分(AC成分)と非脈動成分(DC 成分)で構成されており,これらの比より算出されるRを用いた回帰式よりSpO2推定を行う.AC成分の算出において,信号包絡線による脈動振幅を用いた方法と標準偏差による信号のばらつきを用いた方法の2つの手法を比較し,非接触式計測において,SpO2の低下によるRの増加を定性的に確認でき,非接触でのSpO2計測の可能性を示した., 公益社団法人 日本生体医工学会, 2022, 生体医工学, Annual60, Abstract, 239_2-239_2, Japanese, 1347-443X, 1881-4379, 202202252279426289 - ドップラーレーダによる呼吸・心拍信号計測のためのニューラルネットワークを利用した体動検出

心拍・呼吸などのバイタルサインは,人間の健康状態を判断できる重要な指標である.非接触型の計測方法としては,ドップラーレーダを用いた非接触計測手法が提案され,接触型より簡易かつ長時間計測が可能という利点がある.しかし,同手法は不規則な体動アーチファクトによって,レーダの受信信号に心拍・呼吸情報が計測できない問題点がある.本研究では,フィードフォワードニューラルネットワーク(Feedforward Neural Network, FFNN)を用いて,不規則な体動信号の検出法を開発した.FFNNの体動判定モデルの作成には,心拍・呼吸・体動を模擬したシミュレーション信号データセットを用いてトレーニングを行った.また,FFNN100×20のNN構造でのテストデータセットで検証した結果,体動変位が5~10mm, 10~15mm, 15~20mmにおいて,体動区間の検出エラー率がそれぞれ14.1%,10.3%,7.6%であり,体動変位が大きくなるにつれて,エラー率が低下する傾向を示した., 公益社団法人 日本生体医工学会, 2022, 生体医工学, Annual60, Abstract, 177_2-177_2, Japanese, 1347-443X, 1881-4379, 202202232746213065 - Non-contact prefrontal cortex temperature estimation method trained by a deep body thermometer

我々は精神負荷課題を課した際の自律神経活性を反映する心拍数変動指標や心拍数の変化が健常者とうつ病患者で異なることを利用して,うつ傾向の有無を客観的に判定するための研究を行い80%のスクリーニング感度(Sun,Shinba,Matsui, Front Psychiatry 2016)を得た.更なる感度向上を目指し,加齢による自律神経機能の変化に着目して,精神負荷に対する自律神経応答を用いた年代別うつ病スクリーニング法を開発した.精神科を受診した抗うつ薬未服用のうつ病患者50名(43±13歳),および健常な男女47名(41±12歳)を対象に,それぞれ100秒間の課題前安静,乱数生成課題,課題後安静の心電図から,毎秒,過去40秒間のR-R間隔の時系列に周波数解析を施し,HF,LF/HFを算出した。スクリーニングにはロジスティック回帰分析(LRA)から得た判別式を用い,説明変数には乱数生成課題,およびその前後のHFとLF/HF,心拍数を用いた.うつ病患者と健常者を,20歳代以下,30歳代,40歳代,50歳代以上の年代ごとにLRAを行い,各年代のスクリーニング結果を統合した結果,感度94%,特異度93%,陽性的中率92%,陰性的中率93%の結果が得られた.提案手法は初診時の診療補助,プライマリ・ケアの一環として有望である., 公益社団法人 日本生体医工学会, 2019, 生体医工学, Annual57, Abstract, S52_2-S52_2, Japanese, 1347-443X, 1881-4379, 201902238507806421 - ToFカメラを用いた肺機能検査測定システムによる呼吸機能の非接触モニタリング

肺機能検査は換気能力や肺病の有無を調べる呼吸器検査の1つであり、スパイロメーターを用いて計測する。同装置は、高価であり、取り扱いが簡便でないため、臨床外来ではこれを用いる機会は少ない。また、スパイロメーターは接触測定であること、測定に手間がかかることが課題として挙がる。本研究では、ToF(Time of Flight)カメラを用いた呼吸機能の非接触計測を提案する。ToFカメラは撮像対象までの距離を算出でき、胸郭変動と呼吸変動に相関関係があるため、ToFカメラで時間変動に応じた胸部の変位を測定して呼吸をモニタリングする。非接触でToFカメラによる呼吸機能の計測性能を評価するため、安静時の呼吸数と肺機能検査の肺活量測定の指標である1秒率(Tiffenaeu,Gaensler)を算出した。リファレンスとして呼吸バンド・スパイロメーターと同時計測を行い、記述統計・Bland-Altman解析により結果を評価した。呼吸数算出は計測誤差1.66%、Bland-Altman解析の95%信頼区間は-4.31~3.92bpmの範囲となった。肺機能検査は計測誤差がTiffenaeuの1秒率が15.1%、Gaenslerの1秒率が7.82%となった。計測結果から、ToFカメラを用いて高精度な呼吸計測が可能であることを示した。, 公益社団法人 日本生体医工学会, 2019, 生体医工学, Annual57, Abstract, S239_2-S239_2, Japanese, 1347-443X, 1881-4379, 201902282393831876, 130007776727 - ドップラーレーダを用いた連続非接触血圧測定

近年、世界的に高血圧が問題であり、連続血圧測定の重要性が高まっている。先行研究ではカフレス連続血圧測定の方法として脈波伝搬時間(PTT)を用いた方法が多く研究されている。しかし、PTTを取得するために体の多箇所にPPG(photoplethysmography)やECG(electrocardiogram)を設置する必要があるため、行動の制限や不快感を覚えるなどの欠点が存在する。これより、本研究ではドップラーレーダを用いた非接触による連続血圧測定方法の研究を行った。我々は、心拍1回分の周期と心臓が収縮している時間を用いて、血圧を推定する方法の研究を行った。拡張期血圧(DBP)は、心臓が収縮している時間を用いて推定し、脈圧(PP)は、心拍1回分の周期および心臓が収縮している時間を使用して推定した。収縮期血圧(SBP)は、推定したDBPとPPの和によって推定した。推定した血圧値と血圧計で取得した血圧値を用いて相関係数を算出した。相関係数はそれぞれSBPが0.79、DBPが0.88、PPが0.81と共に十分高い値が得られた。ドップラーレーダを用いた非接触方式による血圧測定の可能性を得られた。, 公益社団法人 日本生体医工学会, 2019, 生体医工学, Annual57, Abstract, S258_2-S258_2, Japanese, 1347-443X, 1881-4379, 201902249005605005, 130007776774 - マイクロ波ドップラーレーダを用いた非接触バイタルサイン計測のための不規則な体動除去法

人体に触れずに呼吸数と心拍数を計測する方法として,近年マイクロ波ドップラーレーダが注目されている.しかし,呼吸と心拍による微小な体表面の振動の他に不規則な体動によって,レーダの直交IQ信号にノイズが生じてしまい計測精度が悪化するという問題点がある.そこで,本研究ではレーダと測距ToF(Time-of-Flight)カメラを融合することによる不規則な体動除去法を提案する.IQ信号とは体動による相対的な変位を直交位相とする余弦波(I信号)と正弦波(Q信号)のことである.ToFカメラで不規則な体動を計測し,スケーリングを施し,これを位相とする余弦波と正弦波を生成した.加法定理を用いてこれらとIQ信号を組み合わせることで,IQ信号の位相内に含まれる不規則な体動成分の除去を行った.実部をI信号,虚部をQ信号とする複素復調を施し,周波数解析を行うことで呼吸数と心拍数を算出した.健康な成人男性5名を対象として,仰臥位,座位,立位の3つの姿勢で,体動を含んだ状態で呼吸数・心拍数の計測実験を行った.呼吸数の平均平方二乗誤差率は,体動除去処理を行うことで仰臥位において19.4%,座位において13.3%減少した., 公益社団法人 日本生体医工学会, 2019, 生体医工学, Annual57, Abstract, S67_1-S67_1, Japanese, 1347-443X, 1881-4379, 201902248504561752 - マットレス下に設置した2台のドップラーレーダーを用いた非接触・血圧モニターの開発

わが国では史上例を見ない速度で高齢化が進展し,健康保健制度が財政的限界に近づきつつある.厚生労働省では往診保険点数の見直しにより在宅医療の普及を推進している.在宅医療患者の感じる不安として,急変時の不安が挙げられており,ショック(血圧の急激な低下)などの病態の急変に備える必要がある.本研究では,安静時の収縮期血圧のみカフで求め,就寝時の一拍ごとの血圧変動を非接触で推定するシステムを開発した.非接触・血圧変動モニターはベッドのマットレス下(胸部(第4肋間付近)及び膝窩部付近)に設置した2台のドップラーレーダー(24GHz 10mW)により構成される.非接触で求めた脈波伝播時間,脈波振幅,心拍数より重回帰分析を用いて,一拍ごとの収縮期血圧の変化を推定し,安静時にカフで求めた収縮期血圧を初期値とし,2分間の静的ハンドグリップ運動負荷(最大握力の30%)時と,運動負荷終了後4分間の一拍ごとの収縮期血圧を非接触で推定する.健康な男子大学生10名(23±1歳)を対象に提案システムによる収縮期血圧の推定値(交差検証)とカフで求めた収縮期血圧とを比較し,推定精度の検証を行ったところ,相関係数0.74 (p<0.05),平均誤差9mmHgの結果が得られた., 公益社団法人 日本生体医工学会, 2019, 生体医工学, Annual57, Abstract, S69_1-S69_1, Japanese, 1347-443X, 1881-4379, 201902256695298099 - サポートベクターマシンを用いた精神負荷と呼吸統制時の心拍変動による精神疾患患者のスクリーニング法

心拍変動(HRV)指標は,交感神経および副交感神経活動を定量的に示し,うつ病を客観的に診断するバイオマーカーとして有用である.数字をランダムに発声させる軽い精神負荷状況で,うつ病患者はHRV指標に特有の反応を示すことが知られている.そこで本稿では,ニューラルネットワーク(NN)を用いたHRV指標による客観的なうつ病診断手法を提案する.NNは非線形判別が可能であり,個人差が大きいHRV指標を扱う点で有効である.静岡済生会病院のうつ病患者44名と健常者47名を対象とし,安静期・精神負荷期・回復期のHRV指標および心拍数を計測した.これらの指標をNNの入力とし,出力値から健常者とうつ病患者の判別を行った.NNの教師データとして臨床経験30年以上の精神科専門医の診断結果を用い,誤差逆伝播法による学習を行った.性能評価では,交差検証を用いて,うつ病患者の数だけNNの学習を繰り返し,その都度評価した.この結果から,未学習データに対して約76%の精度でうつ病患者を判別できることがわかった.また,誤判別したうつ病患者のHRV指標をみると,健常者が精神負荷に対して示す反応と類似していることが確認された.したがって,うつ病患者と健常者をより正確に差別化するHRV指標が必要である.今後は,うつ病患者の特徴量を増やすために,精神負荷に加え,副交感神経を刺激する呼吸統制を導入する., 公益社団法人 日本生体医工学会, 2017, 生体医工学, 55Annual, 4PM-Abstract, 353-353, Japanese, 1347-443X, 1881-4379, 130006077032 - Estimation of Autonomic Nervous Activity for Communication Robot

The non-contact measurement of the respiration rate (RR) and heart rate (HR) using a Doppler radar has attracted more attention in the field of home healthcare monitoring, due to the extremely low burden on patients, unconsciousness and unconstraint. In this study, we propose a time-domain peak detection algorithm for the fast acquisition of the RR and HR within a breathing cycle (approximately 5 s). Signal pre-processing using an analog band-pass filter that extracts respiration and heartbeat signals was performed. Thereafter, the HR and RR were calculated using a peak position detection method. As a reference of HR and RR, the persons wore contact sensors i.e., an electrocardiograph (ECG) and a respiration band. Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research, The University of Electro-Communications, Grant-in-Aid for Scientific Research (C), Principal investigator, 本研究では,3Rs (Reduction, Replacement, Refinement)理念の推進に貢献すべく,洗練された非接触バイタルサイン計測技術の開発により,最大限に動物の苦痛を減らすことを目的とする.小動物の呼吸と心拍由来のmmオーダーの微小な体表面の変位をマイクロ波レーダで計測し,新たな非接触型生体情報モニタリングシステムを実現する. 今年度の研究進捗として,ハードウェアの面では,24GHzマイクロ波レーダを採用し,信号取得のための処理基板を設計し,デジタル信号を収集するシングルボードコンピュータRaspberry Piから構成した.実験動物体表面変位が約ヒトの1/10で0.03~0.7mmであることから,2μm分解能のレーザを用いてイソフルラン麻酔下のラットを対象に,原理検証のためのプロトタイプを試作した.加えて,リファレンス用のレーザ,心電計と動脈カテーテルの導入を検討し,別体であった制御部を本体一体型とし,設置面積を低減,汎用性を向上させた装置を開発した.ソフトウェアの面では,実験動物の生体特徴から「超微小変位」・「呼吸・心拍数が速い」の技術課題を解決できる信号処理アルゴリズムを開発し,リアルタイムで瞬間呼吸数を算出することが可能となった.以上の研究成果を,国際論文誌1件や国内外の学会の研究発表を行う予定である., 23K11301

Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research, Kansai University, Grant-in-Aid for Scientific Research (C), 本申請では,マイクロ波レーダーを用いた非侵襲での肺動脈圧変動の推定法の開発を目的としている.そのために,①非侵襲計測に基づく推定方法の理論的構築と推定式の開発,また,②血流・圧変動計測に適した仕様の非侵襲計測システムの試作,および検証のための③Vitro評価系の開発・検証実験と被験者による検証実験,さらに,実際の医療現場における心不全患者への適用による④臨床データ収集,の4点を検討する. 2022年度は,①理論構築と推定法の開発,②計測システム開発,③Vitro評価系の開発の3点を対象とし,一定の成果を得ることができた.そこで2023年度は,臨床評価の前準備のため,開発したVitro評価系の陰圧負荷装置を用いて試験を繰り返し実施し,推定法と計測システムの効果を確認した.陰圧負荷装置により,心臓へ還流する血液量をコントロールすることによって肺動脈圧低下を生じさせ,その間の肺動脈圧の変化を観察した.安全面を考慮し,一般的に行われる負荷量の半分程度としたことから,還流血液量の低下の効果が少なくなり,また個人差が大きく影響したが,結果としては良好な関係性を見出すことができた.したがって,2022度の理論構築や推定法の効果を確認できた. 一方で,予定していた陽圧負荷についてはその効果が見込まれず,また予定していたASV(Adaptive Servo Ventilation)装置の準備と使用については,その使用時の医療的な安全性の課題を克服できず,2023年度は血圧降下の場合のみの効果を確認した., 22K12917

The system was combined with optical and millimeter-wave senors to non-contact measurement of respiration rate, heart rate, body temperature, and blood oxygen saturation. In this study, we designed the prototype of the system; developed digital signal processing methods for measuring multiple vital signs and machine learning based algorithm to discriminate infected patients. First, theoretical considerations of an estimation method based on noncontact measurements were carried out to develop two estimation formulas. Next, based on the results, a prototype system using a single microwave sensor was developed. To verify both estimation formulas and the prototype system, an in vitro experimental system was developed in which a verification experiment was conducted. Then, in vivo experiments were performed with the cooperation of the participants to verify the two estimation formulas and prototype system. Through these validation experiments using the prototype system and our developed estimation theory, we achieved positive results regarding the estimation of relative change in blood pressure in the optimal blood pressure range., 19K12884

However, some studies indicate that fever-based screening at early-stages of infectious diseases is limited due to many factors that can affect thermographic measurements, such as antifebrile intake. We propose an infection screening system that can rapidly and accurately perform medical inspections. Heart and respiration rates are determined using a medical radar by noncontact way, and body temperature is monitored by thermography. By using these three parameters, the detection accuracy of the system improved comparing to conventional screening method. In this study, to further improve screening performance, one of the most promising approaches is to connect multiple infection screening systems, which enables information sharing between different systems. We developed a portable and a desktop type systems which measure respiration rate, body temperature and heart rate. By pressing photoplethysmographic sensor of the portable system to patient's carotid artery, it measures heart rate, its integrated infrared thermometer measures body surface temperature, and 24GHz Doppler radar measures respiration rate. We evaluated the performances at general clinic and elderly care type hospital. Our system could measure the vital signs within 10 seconds and discriminated infected and non-infected using machine learning algorithm. At first, theoretical consideration of a method for estimating the blood pressure dedicated to non-contact monitoring using microwaves was initially conducted. To confirm the effectiveness of this new method, the prototype system for verification was subsequently created and then verified in laboratory experiments. Furthermore, to accumulate clinical knowledge and determine current issues associated with this method, clinical trials including patients with heart failure were planned. We achieved positive results through validation experiments using the prototype system implementing our developed estimation theory. Unfortunately, the clinical trials were not possible because the safety of patients with heart failure could not be ensured sufficiently. The clinical trials remain as a future task., 16H03215
